Facial pressure that won’t let up, thick nasal drainage, and a nose that stays stuffed for weeks can be exhausting to manage. Allergies or a lingering cold seem like the obvious explanation.
Sometimes that’s exactly what’s going on. Other times, these symptoms point toward a sinus infection that needs more than time and over-the-counter medication.
Common Symptoms of Sinus Problems
Sinus problems can present in several ways. Difficulty breathing through your nose, mucus or discharge, and facial pressure or fullness are among the most common signs.
Some people also notice changes in their sense of smell or find themselves getting sinus infections more than three times a year.
Postnasal drip, a lingering cough, and bad breath can be secondary symptoms, especially when drainage runs down the back of the throat.
Many of these symptoms overlap with conditions unrelated to the sinuses, like migraines or jaw tension. That overlap is part of why timing plays a role in deciding what to do next.
When a Primary Care Doctor Is Enough
For a short-term issue, a primary care provider is usually the right first stop. Most viral sinus infections start improving within about ten days on their own.
If symptoms haven’t improved after a week, a bacterial infection may have developed. A course of antibiotics from your primary care doctor is often enough to resolve it.
Signs That Point to an ENT Specialist
Two patterns suggest it’s time to see an ENT instead of continuing with primary care alone.
The first is duration. Symptoms lasting 12 weeks or longer point toward chronic sinusitis, a condition that typically calls for a specialist’s evaluation.
The second is frequency. Getting acute sinus infections four or more times a year, sometimes called recurrent acute sinusitis, suggests an underlying issue worth investigating.
What Might Be Causing Ongoing Congestion
Ongoing congestion sometimes traces back to a structural cause rather than a simple infection. A deviated septum, where the wall between your nostrils sits off-center, can make one side of your nose harder to breathe through.
Nasal polyps, soft growths inside the nasal passages, can also block airflow and contribute to a chronic stuffy feeling.
Environmental allergies and ongoing exposure to irritants like smoke or pollution can keep the sinuses inflamed even without an active infection.
When a deviated septum or nasal polyps block normal sinus drainage significantly, surgery to correct that structural issue is sometimes recommended alongside other treatment.
Emergency Warning Signs
A small number of sinus symptoms call for immediate medical attention rather than a scheduled appointment. These include a severe headache, noticeable swelling around the eyes or face, vision changes, a high fever, neck stiffness, or sudden confusion.
Those symptoms can signal a more serious complication and warrant a trip to urgent care or the emergency room.
What to Expect at a Sinus Evaluation
A first visit with a sinus specialist usually starts with a detailed history of your symptoms and any treatments you’ve already tried.
Many ENTs also perform a nasal endoscopy, a brief exam using a thin scope to view your nasal passages and sinuses.
Depending on what that exam shows, your specialist may recommend allergy testing or a CT scan to get a clearer picture before discussing treatment.
Follow-up visits help track whether initial treatment is working before considering more involved options.
Treatment usually starts conservatively, with daily saline rinses and a steroid nasal spray to calm inflammation. Antibiotics or sinus surgery become options only if those initial steps don’t bring enough relief.
